The Build:

Adrenal Converter enchants pop on a self poison activation. Once we get our Spider Maces rolling, they immediately turn into near infinite activators (1 Slow from Adrenal Converter, 2 Poison from Toxin Injector, 1 Poison from self).
We took extra care to keep Adrenal Converter as our only regen item so it gains all of the boosts from the regen skill (I can’t find the name, when you Poison/Burn/Slow/Freeze, a regen item gains +2 regen). This makes it so every poison activation gives slow on an opposing item, haste, and scaling regen.
Toxic (when you poison, your weapons gain +15 damage) gives us the scaling damage to close out the fights.
Our poison stays exceptionally low with lifesteal taking away the self inflicted damage.
